First Value in dependent choice list field is not selected on first time

We have dependent choice list field 'Rejection reason' created on approval table which is dependent on task approval type. 

We have configured choices for the same with dependent value. 

We are able to see the choices in the 'rejection reason' choice list but when we select first choice in the list it is not getting added into the list. However, when we choose second choice add it and then try adding first choice then it is getting added. 

We are having workaround, but wanted to check if anyone encounter the same and have solution OOTB.
