Catalog item in Now mobile app

I Want to create an input screen for a specific item in now mobile app
If I use the link it is taking too much of time to load 
Any one help

and I have a flow for the service catalog item 
If i am using rimt table for input screen flow is not getting triggered.

 I am unable to auto populate values in input screen 
 based on the selection of the group
I want to populate the group members
