discovery run creates duplicate entries in hardware model table, IRE config??

There are duplicates generated on discovery run for the Hardware Model table with multiple entries on the MODEL ID and MANUFACTURER. There are duplicate CIs created with only IP address and name as IP address in the CMDB CI table. What is the criterion to update the CI Identifier  for the hardware table?? or is there any other field to work upon?
