Upgrade Details capture in updateset

Hi,

I was trying to capture in an update set my comments and resolution in the Upgrade Details but I don't know why some or most of the updates I made wasn't captured in the update set. Out of 500 records from Upgrade details table only 5 was captured. 
Can anyone enlighten me on this matter?
