After washington upgrade custome UI action not working

After washington upgrade custome UI action is not working, I checked for other instance which is not upgraded for that its working properly, checked condition and script everything is same on both the instances, also tried deactive and activate but nogo.

Any suggesion will be appreaciated.

Already tried this but nogo.
UI action is visible but after clicking on it nothing happening.
