Automated Response in Smart Assessments

I have written script in default response of automated response in Smart assessment question. The default response script is appearing thrice in UAT but in DEV it is visible only one time. What could be the possible reason for that? Ideally only one default response script should appear.
Also, is there any backend table where all the response of automated response script are stored?
