Calculate risk based on questions

Hi All, 

Need to calculate the risk based on questions,

tables involved to store questions and scores to show the e.g "take assessment" UI action for change form.

I got few links on community and also did some R&D, but your suggestions would be helpful.

Plz suggest simple way to do that.
