1. Create a template so that Incidents created from Service Catalog or a Portal using the record producer "Report Performance Problem" should be assigned to the "Service Desk" group.
2. Using Assignment Lookup Rules, make sure that Incidents created against CI: PS ORA01 are assigned to the group: Database
after completion of task error showing.

For Q1, you need template to be created as show below and then map same template on record producer under "Generated Record Data" section.
Since validation error message is only about template so I believe your Q2 is correct.

As your template has "assigned to" set not "assignment group" as per requirement. Take a small break and start your work which helps any developer to rethink what is done and what are we doing.

For Q2,go to filter navigator and search for Assignment lookup rules -> Click "NEW".
Now select
Configuration Item as "PS ORA01"
Assignment Group as "Database".
