Ctask record is closed without mandatory fields filled

Hello,
Can anyone please let me know the exact answer i searched in community and googled it but i didn't find it.
I have a Change request and Ctask is created for it. Ctask is closed but it is having mandatory fields which are not filled. What are the possibilites that a record can be closed without providing mandatory fields

3 things to check
-> Possibly that record got updated from list
-> Possibly that record got updated via script where mandatory check doesn't happen
-> Possibly that record got updated via script using setUseEngines(false) and it will skip data policy if that data policy is making that field mandatory.
