How do you address the failed change?

I wanted to unerstand how do you manage the failed change and any lesson learned? Do you utilise via the Post-Incident review or do you have a separate function within your change management where the change manager would liaise with the Change coordinator to fill out the Post-Change Review questions to understand what has happened and any lesson learned out of it.
