How to insert record containing date to table using form in UI builder?

I am using serviceNow UI builder, and tried to create new record in table "CommitteeHierarchy" using form bind with the same table and button triggering the event "create record". It works if all field values are string format. However, it fails if a field with date format is included. How to fix the problem?
