Incident being created to use decision table

Currently incidents are being created by record producer. The new requirement is to continue using the record producer but implement a decision table that would using the following fields:
Impact + Urgency = Priority
Issue details and subissues
Based on what is selected from these fields would determine the Category, Sub Category and Assignment Group.
I have created the decision table: see screenshot
But I am not sure how to create the flow to use the new decsion table when an incident is created.
Any assistance would be appreciated.

The question is I can't seem to trigger an incident so that the decision table can be used. Why is that? or how to do I get that to be triggered?

Oki @Roshni1 the trigger is set in the flow:
before you Save/Activate it (top-right corner) you can sue the test button, where you can select any existing incident and you will see how would that incident be handled by this flow.
Or you can Activate and then create incident (from portal or backend) matching the condition from the Trigger part that I just highlighted above.
Let me know if you have any progress, optimally try this for the conditions in decision table - to match and to mismatch to see if it is stable solution..
