Looking for a tutorial to create a post incident report for Problem

Hello Everyone,

 

I am looking for a tutorial or guide to assist me in creating post incident reports for problem. The report needs to pull numerous fields from the problem form and send these to the problem manager once the problem has been closed off, cant seem to find where I can configure this within ServiceNow.

 

Could anyone point me in the right direction?
