Reference field Not populating when selecting record

Hi All,

We have a reference field( which is associated with "cmn_schedule_span"table ) on Custom table form to select Schedule Entry.   When we select the magnifying glass it brings back a list of records as expected, however if you select a name it does not populate it in the field but when I click on the preview record then I am able to see the record information which I selected.  Not Sure why it is not populating the issue is in all the environments.

 

I have verified some of the configuratiopns like display to true in table and verified client scripts and all those but no luck.

Display value as true for any column in given table is only visible:
Navigate to All > System Definition > Tables - Enter the table name as Label Column value:

Another example: If you reference Incident Table anywhere, then it's Number like INC000 is visible because Number column display is true in Incident table as shown below:
