standard change proposal number is changing after approval in servicenow

Hi All,
We have created a standard change proposal. After that, the manager's approval is requested. Once he is approved, the proposal number is automatically changed for random cases. Does anyone have this issue .

Is it just changing the number or creating a entire new record. What's meaning of "random cases" here.

Just change the number alone, not create new records. Random means If we submit 10, it's happening only 2 or 3 records
