Tab not getting created under the interaction tab uponclicking of a Create Service Inquiry UI Action

We have a UI action for a custom table on Service Operations Workspace. Create Service Inquiry. The tab is not getting created under the Interaction tab as shown in the shouldbelikethispng. I activated the Create Incident UI Action also on the form, it gets created under the Interaction tab in the training instance but not in our instance. It seems like there is data (JSON or a record ) that puts the newly created form under the tab in the training instance that I am missing. I have looked into macroponents and page route list but so far not landed on a solution yet.

Attached is the UI action i am using and the resultant tabs, the create incident ui action is the same as the service inquiry except the table name is service inquiry.
