I'd like you to let me know whether you've set up something that'll prompt a PCR Task, if a Change Request is not successful? So, if any of the associated tasks are not closed as Successful, then, it'll generate this Task.
So, questions like Backout Performed (potentially a button), however, I'd like to have a drop-down to further question whether the Backout was successful etc., then, additional free text fields such as:
Summarize Issues that were caused as a result.
List any risks to keeping it in current state.
What are the direct and indirect impacts of this Change Request?
What are the findings and lessons learnt?
What are the follow up actions?
