field not Autopopulating Service Operations Workspace on incident form

Hello All,
I created an incident form on the SOW and would like for the department field and company fields to auto populate when the customer name is entered

Finally got a chance t go back work on this a bit and wondered why my Client Script doesn't work on Agent Workspace yet on the platform. I ended up setting the UI type to "all" and that took care of the issue.
