Not set value from Employee form(Collect Employee Form) to HR case

Hi All/ @Sandeep Rajput ,

In our instance we have seen Employee Form collect input but those field value not populated correctly. It is not happening for every cases but some case we have see. 
If we see "asmt_assessment_instance" table and open the record and we see all value is there according to Employee form, however not populate few of the field mappings in HR cases.

What is the probable reason ?

@abirakundu23 Employee form is not appropriate in this case, you should use a record producer with variables to take input from the employee and the HR Task type should be Submit catalog item on the HR Template.

Hi @Sandeep Rajput ,
In our instance already used onboarding record producer with variables to take inputs when case created from Portal, for the same service HR case created for onboarding through integration an collecting those information through Employee form and set in HR case according to mapping. In that case what be the approach to avoid  such a issue . Please assist. 

@abirakundu23 In this case, you need to make the condition unique so it returns a single record. Try to see if you can use dynamic filters in the condition builder to return the unique record.

Actually i am trying to make condition in such way to return unique record, but its returning more than one record.
Could you please assist ?
