What is this Blue Notebook showing up on the Description field in a list view?

It doesn't seem like ServiceNow much.. Where did you get it, Portal or backend?

either way check for the Field Styles:

[Mark Endsley]

I assumed it was OOTB, turns out we added it (as a field style) to indicate there are attachments on the record.
