It doesn't seem like ServiceNow much.. Where did you get it, Portal or backend?

 

either way check for the Field Styles:

It’s a symbol that is tagged to this field. You can check the Dictionary Entry or the Dictionary Override for the same.

I assumed it was OOTB, turns out we added it (as a field style) to indicate there are attachments on the record.
