Duplicate Tasks are being created with nothing to remediate

We are receiving new duplicate tasks created when there is only one active CI and the other record is empty.  Why would this occur? One oddity is that a relationship was defined in some classes. I reviewed the CI with the relationship noted, but nothing stands out. Many are just completely empty, so how is it even finding a duplicate? I am not sure where to investigate.

@Donna Olsen  Duplicate task should have details of the SysIds it identified and what criterion attributes it matched on when it created this task. Can you check the details on it. 

 

If it is empty most likely either its deleted or it just doesn't have display name. Look at the xml record to find the other details of the CI. 

Thank you, but it appears there are empty records with relationships, and I cannot view the XML record of a file that doesn't exist, yet it still shows a relationship on the Duplicate Task itself.

It looks like the CI got deleted, can you check if there is any automation remediation enabled? I would look at the XML of the duplicate task to find what the sysIds are when it created, the task notes should also have the details.
