@Roopa11 Can you please double check assessment instance record, are you seeing record number in "Trigger Id" Column ? if it is coming then it should show in your info message.

Survey triggering once ticket (Incident and RITM table ) closed and for particular assignment group
Survey where its not showing ref number is for ITSM modules.

Yes Harsh,
Trigger ID column value missing.
In CSM instance its populating Trigger ID column value and info message has number.
