populating task details on survey form

i have created a survey which will trigger when the incident or request will be moved to resolved state . my requirement is to populate the incident or  request details to which the user is giving the survey on the survey  form itself  how can i achieve this 

this is how i modeled it  but now i want the respective incident details to be populated to which this incident is triggering on the form itself . how can i acheive it

@bradleydebono I have what I think is the same question the author posted, wondering if you circled back to it. How can you put the incident information on the survey. Your solution below appears to be putting survey information on the incident. So more specifically, I want a survey question to say "Did we resolve INC0010010?"  The actual incident number on the survey form. It's easy on the email invitation, but is it possible to put in the survey.
