After the record producer submitted, the attachment showed error

hi all,

 

when i open a record producer form, there is an attachment variable, i upload an attachment, then i delete it, then i upload an attachment again, then submit the form. on the case ticket page, activity log tab, will show two duplicate attachment. 

It will also display the attachment I deleted, but when I refresh the page again, the attachment I deleted will disappear.

I want to know what causes it and whether these two problems can be solved.

is this happening on all records which are generated from record producer?

Did you check for some other record producers as well? If it happens the same there then it's a platform feature/bug.

Yes, other record producers have this problem as well, and so do other instances.
