Assessment template questionnaire to display questions dynamically

I have a section in my assessment template which has a set of questions that needs to filled multiple times 

If I need to fill in details for 4 sets I want to display 4 questions dynamically 
I didn't find any OOB and for now I have hardcoded 7-8 sets to show every time I want to have a control on how this is done 
I am creating my assessment template using Risk assessment designer
Can we try applying and client scripts or UI policies , If yes on which table and how can we do that .
