I am completely new to service portal. I need to submit a catalog item from service portal. To achieve this I used Open a form (SP) step but I am getting an error "g_form is not defined. A valid form must have g_form defined". Please suggest what this error indicates.

As per your suggestion I have modified the changes but here also I am getting error as below.
Failed to open Catalog Item. Either you don't have access to the Catalog Item or g_form is not defined
Any suggestions!!!

Does your user have access to the given Catalog Item? Is it active in the Catalog?
Can you please provide the screenshot of the Open Catalog Item step as well as the result record?
