Attach survey responses to the target ticket

Hi All,
I have a survey created and i'm trying to attach the survey responses given by the user to the target ticket.
Can any one let me know how can this be done.

Hi Nikhitha,
Any particular reason to attach the response to the target ticket? At any point you can check for asmt_assessment_instance table and understand the target ticket. Not sure, why there is a need to attach it to the ticket as I feel survey will be for assigned to and if fulfiller will have access to ticket. Ideal, will be to keep survey response anonymous or hidden from agent working and by attaching respone that confidentiality will go off.

I am agree with @Jaspal Singh , if you bring the survey to target table it will be visible to agents and can impact the productivity. Also, as it is client ask, we as SN experts can tell plus and minus to bring this change.
