Hi, have you confirmed that the attachment had\has a valid name when it was created\uploaded?
You could also check the updated on and updated by time stamps of the sys_attachment record, if these are different from the created by\created on details then it is likely that someone edited\removed the name from the UI?
Otherwise without details of how the attachment was added (or created) it is not possible to provide an assessment from a partial screenshot. If my response hasn't helped you identify the cause of your issue then perhaps you can update this thread with some more specific details around the attachment creation process.
