Catalog Item Attachments are all showing up on SCTASK after Vancouver Upgrade

Please guide on why all attachments attached to a catalog items are showing under SCTASK related list "attachment" tab, when a user is only submitting just one of those attachments.
This issue surfaced after Vancouver upgrade.
Please guide if I can create a Business Rule to allow only the user submitted attachment to show on the SCATSK rather than all the attachments attached to catalog item.
Any help is appreciated. Thanks.

The "attachment" related list sounds like a custom relationship. Post the script for that if it is. Are you saying prior to Vancouver if the Requested Item record has 3 attachments, this Related List would not show them unless the (current?) user is the one who attached them to that record?

Yes Brad! Prior to upgrade, if a user attached an attachment from the multiples that were part of the catalog item, only the attached one will show under the SCTASK related list "attachment " tab. Now, after the upgrade, all the ones attached to catalog item plus the one user submitted are showing. Any clues??

Only the highlighted one was selected from 3 attachments in catalog item while submitting the request, but it is showing total 3 attached to catalog item plus the one attached by the requester as duplicate. This screenshot is from SCTASK form related list.
Any help is appreciated.

Navigate to System Definitions >> Relationships and check for Name: Attachments Relationships to understand the query it uses to show results.
