Completed Risk Assessment doesn't update risk on the change request record

Hello,

1. I've created a new risk assessment form for Hardware Change Requests

2. I deactivated all Risk Conditions (the customer doesn't need them)

3. When I complete the Risk Assessment form I get this:

Based on my answers, the risk was correctly evaluated to 'High', however, the record was not updated. Why?

Edit: Wrong suggestion to solving the problem was given by mistake. I have removed it to avoid confusion.

My "change_risk_asmt" record doesn't have the "Update field on assessed record" setting or am I looking in the wrong place? 

FYI, I did the same as I described above and it worked on my PDI.

I seem to have remembered incorrectly. Is the change risk property set to UI Action? 

You should be getting this: 

And this when you have finished the risk assesment:

Yes, it is set to the OOTB value ui_action. Risk assessment evaluates correctly (high), however it doesn't update the record (risk is still moderate)
