Hello,
1. I've created a new risk assessment form for Hardware Change Requests
2. I deactivated all Risk Conditions (the customer doesn't need them)
3. When I complete the Risk Assessment form I get this:
Based on my answers, the risk was correctly evaluated to 'High', however, the record was not updated. Why?

Sounds like it could be a business rule that is causing the record to not be updated? I'm guessing it would be a before-businessrule in this case. Have you tried to compare the business rules on the PDI with the business rules on your instance?

I don't think it is related. I just verified in the PDI that it still works as expected with a user who only has the itil role.
