Contextual search button

Hi all,
my requirement is like to enable contextual search button in front of short description field on incident form how can we get the button. For reference I have attached screenshot(Yellow button) also same button available on change form and RITM form.

OOTB contextual search does exits for short description but if you want to check then do this :

Navigate to contextual search>>Table Configuration

In list view select 

In this record select a related list 

To add the "Contextual Search Results" element to the incident form, please follow the steps below:

1) Log into your instance and navigate to Incident > Create New.

2) Right click on the form header and choose Configure > Form Layout.

3) Choose the "Contextual Search Results" option from the Available list, and add it to the Selected list so that it appears just below the "Short description" field.

4) Click the Save button.

Please look on attached image, i need to get Yellow marked button on incident form next to short description.
