Decision response coming as null in the script include

I'm trying to populate a reference field in the record producer form called "Assignment Group". This value should be populated based on the selections of three other fields in the form.

I have created a decision table with 3 inputs which is of type reference (as these are reference fields in the form). There is one decision as of now which has got the values. The input values are coming correctly in the script include, but the response is coming as null.

Any suggestions, please
