Change request form fields Planning start date& end date, justification, implementation plan, risk and impact analysis, backout plan how to add in email notification. please help friends.

I believe once you open the identified notification template record.

In the email body section, you can use variables to reference the fields you want to include. Variables are usually enclosed in double curly braces {{ }}.

For example, to include the "Planning Start Date" field, you might use: {{change_planning_start_date}}.
