Change request form fields Planning start date& end date, justification, implementation plan, risk and impact analysis, backout plan how to add in email notification. please help friends.

In the same manner. if you see in right side, fields are there. Just click on desired field and it will be added in email body.

You can create new notification from notification module and table change_request table and in what it will contain tab expand on fields and add necessary field to body.
