Error in service portal

Hello developers, im getting Error when adding multiple requests from cart. im getting error as "Violations against unique key detected by database conn=2******) Duplicate entry 'dee7af7ff******' for key 'group 2'). Could anyone help me the reason for this issue. Any help is appreciated. Thanks!

Hello @Ankur Bawiskar : Thanks for your quick response. I tried to get the script as the issue in in PROD env couldnt replicate the same in lower instances. Thanks !
