Event fired but notification not sent

We have a notification set up that isn't sending when the event is fired. This is the set up for the notification:

My workflow shows that the event was triggered:

I found the event in the event list:

But, the notification was not triggered. I see other notifications, but not this one (or the other event for reminder to the opened by and requested for that is triggered through this workflow).

I'm not sure what is wrong, any help is greatly appreciated!

Try giving "0" as weight and try.

Hi Arnab - thanks for the tip. The weight was 0 when I originally tested and cleared it out because I was comparing to another notification that was working and it had no weight. I'll add that back in, but probably not the source of the problem.

If you are using Helsinki (possibly Geneva), go to your notification record and click the Preview Notification and you can test it with a record/event and see who would receive it. If their name is crossed out, hover over the name to find out why. It's a great way of debugging notifications.

Hi Chuck - actually, when I preview this notification absolutely nothing comes up. This is what it looks like:
