Flow Designer: Individual custom action works standalone, but not in Flow

Hi all,
I got a funny issue and need some input. I created a REST API custom action in Flow Designer to fetch the Associated Site Groups from Sharepoint online. I got the spoke set up, the application setup working, can get tokens and do REST calls from backend. When I test the action stand-alone, it works fine and comes back with the correct groups.
When I call the action from within a flow with the same input, I get a 403 "Attempted to perform an unauthorized operation." response.
I tried copying the flow from Global into SP Online scope, but the same thing happens.
Out of ideas.... Hoping I am just dense today.

Hi Bjorne,
Somehow, I'm facing the same issue recently, its not like it didn't ever work but it worked initially and then when I changed something in the child action and rerun the flow designer, it never worked after that.
Please let me know if you have come across a solution.
