Flow Designer not triggering for catalog item request

Hello,
I am working on a catalog item in ServiceNow where I have configured a Flow Designer flow to trigger on record creation. However, when I submit the request, the flow does not start automatically.
I have verified:
Flow is active.
Trigger conditions match the record.
Correct table is selected.
Is there anything else I should check? Any common misconfigurations that could cause this issue?

Hello @ThanuRajM
Have you checked that your REQ records is approved or not? As there is ootb workflow which auto approves request and eventually then flow get triggered due to one ootb BR, I don't recollect name but this issue is mostly common.

Have you tested this flow from flow designer itself and check the context to understand on which step it got stuck.

Please share flow config/trigger screenshots.
Your flow should trigger with "Service Catalog" trigger
Things to check
1) is flow published and added to that catalog item in process engine
2) is your REQ auto approved after submission or it's in pending state? If it's auto approved then Flow should kick off
3) your instance should have only 1 Workflow on sc_request table. Hopefully it's not deactivated or not deleted.
