Form fields on portal showing blacked out

Hi Folks,
I have added form view on the portal , but few fields are showing blacked out as below. Has anyone encountered such issue ?

Hi @Tapish Sharma1 ,
1) Please check if the fields in questions have been configured as read only in the form layout.
2) Check the UI policies or Client scripts on the form making field read only according to some conditions.
3) Check if any ACLs are existing to edit the fields.

Hi Hardik,
Thanks for your response , I dont think this is related to ACL , as I am logged in as admin user, Also when I revert the theme to base theme , this issue doesnt exist, It seems related to something CSS

Seem like the system theme for the string input field has been modified to have that background color
Inspect the element on browser console and check the class and verify that class in the Service Portal > Theme record of the current portal or it might be also done on the page level css or directly on the widget code level

Did you check in some other browsers?
seems some issue with theme or are you having some information pre-filled?
