Generates a report of standard change templates

Hello everyone

I need to generate a report of the standard change templates containing all the template information, name, description, etc, and the change request values: Implementation Plan, Rollback Plan, Test Plan, Assignment Group.

I have some of this information from a view in the std_change_record_producer table, but not the Implementation Plan, Rollback Plan, Test Plan, Risk, Impact Analysis, etc.

How can I generate a report with all this information?

I really appreciate your help.
