How should I add a form in service portal

I created a form in the platform side. how should I embed the form into a service portal page. can anyone help me.

What kind of form have you developed?

 

Is it a Catalog Item\Record Producer or a Normal record form for a custom/OOTB table?

 

If its a Catalog Item\Record Producer then it will be by default available in the service portal. You can search in the global filter with the form name it will appear or under request something.

I am still in the design part, but I think I will go with

So when i say a custom/OOTB table normal record form it means like how we have a incident record. Its a OOTB table correct & when u open a specific record the data which we see is a form view. 

 

So if you want to display that on portal, the next question would be like when u want that form to be viewed on portal. so for eg if I click on something then that form should appear or after clicking on something it should take me to that form.
