How to cal UI Page in to Dashboard or Homepage

Hi All,

I have OOB UI Page  I want to call this UI page in to dashboard can you please giude mw how to do it

UI Page :-

/assessment_scorecard.do?sysparm_record_sysid=13258d49dbcd101092760793e296190d&sysparm_assessable_table=asmt_metric_type&sysparm_metric_type=13258d49dbcd101092760793e296190d&

I created the custom widget like this below and I called it on the dashboard but it's not working
