How to create custom notifications in Preferences

Hi All,
I have a requirement like, 
I need to create a custom notification in Preferences as shown below:

So when i create a custom notification here for a scenario like when the incident priority is P2 and assignment group is ABC then i need to trigger this notification.
When the notification is created here how can we test whether this notification is triggered correctly or not.
