Adding risks assessment against standard changes is not best pratice the reason is, standard changes are pre approved and considered as low risk and deployed any time, if you add risk assessment you need to do changes in OOTB

In Flow as well.

I would like to the business case for same.

You can't add risk question in templates.

Do you have risk question on standard change form ? if yes share the screen shot.

If the CHG form has columns ( variables ) then you can add the same in template.  However all standard changes are pre-reviewed for risk which is default low.

Our change form contains a risk field, however it is read-only. Also, I need to include the risk assignment questions in the standard change templates.
