How i can add the below task :- 

 

I am try to adding Risk Assessment Questionnaire on the ServiceNow Standard Change template form.

I have strong feeling , that we cant add the risk question on Template.

@Vamsi_Y , 

In general, can you edit the risk column value on change form ?

The Risk is drived from some change parameters, and this risk calculation trigger when we move the new change to assess.
