Lookup select box

Hi team 

I am trying to fetch the data of table using look-up select box variable in service catalog.

I could only fetch the data of table or the lable of the table.

I want to configure in such way that is user user backend or the table name it should be displayed.

Example: if you select the cmdb_ci the data need to fetch in the variable or configuration item both need to be displayed in one lookup select box field.

If I understand your question, you want to create a Lookup Select field to show a list of items in a table and store 2 field values from the selected record in the Lookup variable. Is that right? I'm not really understanding how that could be useful, but maybe I'm misunderstanding the question. Could you maybe give some more detail?
