Notification on low survey scores

I've been asked to create a notification for when a survey is returned with a low score. We are using Best Practice - Task Survey Management. Has anyone set up something similar? Thanks for any suggestions!

I have also got the same requirement whereas in my case for negative feedback it should open a Service request.

I created a notification on the Assessment Instance Question table triggering on a question weight of greater than the weight of the response....works great.
