Notification on low survey scores

I've been asked to create a notification for when a survey is returned with a low score. We are using Best Practice - Task Survey Management. Has anyone set up something similar? Thanks for any suggestions!

Create a new Notification on Assessment Instance Question table.

When I setup the Survey, I used Likert type, which has weight values for the responses.

In the notification, just set a condition to trigger when a value is greater than the 'unsatisfied response' value.

I used a template data type, I only want to send a notification if the user responds dissatisfied or very dissatisfied which is the string value but nothing is sending when I test it.  I did test using value instead of string value and it sent 4 notifications which is not what I want.
